Cornerstone Community Services is committed to developing a comprehensive continuum of mental health services to assure every individual receives appropriate services and supports when needed that are guided by the following standards:

OUR FEES
Cornerstone Community Services accepts a full range of insurance coverage including managed care plans, medical assistance, Medicaid, and Medicare. If you do not have insurance, fees are based on your ability to pay. It is our goal to make sure services are affordable to all residents of Bedford and Somerset counties.
WHAT WILL HAPPEN WHEN YOU MAKE AN APPOINTMENT?
When you call our registration department, an initial screening will be conducted over the phone. The case manager will gather information about your concerns and circumstances that contribute to them – treatment recommendations will be based on this information. We offer comprehensive care by working with other community agencies.
Before your first service appointment, the registration worker will meet with you to gather insurance information and complete required paperwork. At this meeting, the cost for services will be determined and the initial appointment will be scheduled. At all times you and your family will be treated with respect and confidentiality.
MENTAL HEALTH SERVICES
Just as diabetes is a disorder of the pancreas, mental illnesses are medical conditions that often result in symptoms that can make it difficult to hold a job, go to school, relate to others, or cope with the ordinary demands of life. The good news about mental illness is that recovery is possible.
Mental health recovery is a journey of healing and transformation to live a meaningful life in communities while striving to achieve full human potential. With appropriate treatment, most people can recover from mental illness and return to normal life. Our programs include the following:

Our Clinical Services Unit (CSU) helps individuals of all ages and backgrounds learn to better cope with the challenges of life. We provide support if you are having problems with:

• Problem-solving • Assertiveness • Mood stability
• Spiritual support
Counseling involves participation in individual counseling sessions. We can also expand sessions to include spouses, significant others, children, and extended family members as needed.
Services include:

♦ Individual, group, and family therapy
Outpatient therapists work closely with community resources and a variety of programs to coordinate care. Day and evening hours are available for all appointments.

PARTIAL HOSPITALIZATION PROGRAM (GCC)
Everyday life can be overwhelming and sometimes people need help coping with their problems. It may not require an overnight hospital stay, but it can be serious enough that routine counseling won’t solve it. Our partial hospitalization programs – referred to as Group Counseling Centers (GCC) – offer intensive day treatment for adults experiencing emotional problems that interfere with their daily living.
This program promotes and encourages hope, wellness, and recovery to individuals and their families in our community.
We offer a variety of therapeutic, educational, recreational, and support groups. Issues discussed include abuse, depression, anger management, coping with loss, family dysfunctions, self-esteem, interpersonal relationships, alcoholism, and effectively managing mental wellness. Treatment is based on an individual approach under the direction of a psychiatric physician.
CONFIDENTIALITY: Group participants are made aware of the principles of confidentiality and the importance of protecting others’ privacy. This understanding increases trust and ensures a positive group experience.

LONG TERM STRUCTURED RESIDENCE (LTSR)
The LTSR is a safe, structured home-like environment for people who need a higher level of psychiatric care 24 hours per day/7 days per week for an extended period of time. The program provides intensive treatment and supervision to meet the special needs of adults with severe and persistent psychiatric illnesses such as schizophrenia, bi-polar disorder, depression, etc.
Our recovery focused and individualized approach targets individual needs and abilities to assure the comfort, safety, health, and wellness of each resident. The program provides longer-term stabilization services to prevent re-hospitalization and promote, when possible, successful transition to less-restrictive community programs.
The program provides meals, 24 hour awake staffing, and a daily schedule of therapeutic and skill building activities. LTSR has a team of medical, therapeutic, and residential staff trained to meet the needs of each individual. Program activities are designed to maximize stabilization/rehabilitation and promote independent living skills to eventually return residents to independent lives.

FAMILY BASED MENTAL HEALTH SERVICES (FBMHS)
The Family Based Mental Health Services (FBMHS) program assists families in caring for children or adolescents with emotional problems. The program uses an intensive family therapy approach designed to maintain and strengthen the family unit. The treatment team, consisting of two mental health professionals, provides services directly in the home and community. Parent and child participation is critical. Therapists work with the child/family to guide them toward identifying their strengths and meeting their own needs as well as the needs of the family.
Treatment is highly individualized and tailored to meet the unique situation of each child and family. Support is provided in the following areas: family/individual therapy, setting goals, improving coping skills, stress management, and parenting skills.
WHO IS ELIGIBLE?
♦ Families with children/adolescents 0 – 21 with emotional and/or behavioral problems.
♦ Families with children/adolescents at risk of psychiatric hospitalization or residential placement.
Admission to the program requires a recommendation in writing from a licensed psychologist, psychiatrist, or family physician.
The FBMHS is a 32-week program; however, upon evaluation the time frame can vary based on your family’s needs. The frequency of visits will depend on the needs of the family. In order to accommodate your family’s schedule, this service is provided during weekday and evening hours.

SCHOOL COUNSELING PROGRAM (SCP)
When children and adolescents face difficult circumstances they may show symptoms of depression or anxiety. At times they act out impulsively or even consider suicide. In these situations, it is helpful to talk with a professional who is attentive, objective, and is able to teach cop
ing skills.
Our School Counseling Program provides therapy in a school setting to children and adolescents exhibiting mental health problems. The program's mission is to ensure that all school aged children, who have emotional problems, are able to readily access needed mental health or supportive services.
Outcomes include improved mood, stronger family relationships, increased academic achievement, improved attendance, social-behavioral skill development, individual planning and family/community involvement.

BLENDED CASE MANAGEMENT (BCM) – ICM/RC
Living with a serious and persistent mental illness can be challenging. Our Blended Case Management (BCM) program helps both adults and children with a serious or persistent mental illness who need assistance in accessing, coordinating, and monitoring resources and services. Support services could include medical, social, educational, financial, recreational, or other services.
Individuals are assigned a Case Manager who provides face-to-face and phone contact to consumers in their home, at work, or in the community. They monitor progress towards recovery and healthier lifestyles and advocate for consumers when needed.
The goal of BCM is to build a network of community resources and services based on each consumer’s strengths. It is the goal that this will increase the quality of life, improve level of functioning, and link consumers to needed services. These goals help consumers remain in their community and avoid hospitalizations and out-of-home placements.

COMMUNITY RESIDENTIAL REHABILITATION (CRR)
The Community Residential Rehabilitation (CRR) program is a community based, transitional residential program for persons 18 years and older who have a chronic psychiatric disability. CRR provides a rehabilitative training and assistance in the skills of community living. CRR recognizes that each individual is unique and strives to provide flexible, individualized service planning according to CSP principals. Different levels of staff assistance are offered to maximize individual potential.
This program is consumer-driven and built on strengths and self-reliance. The program also embraces recovery, employment, education, and community integration so each resident learns the skills necessary to move on to independent living. Rehabilitative services offered include training and supportive coaching in:
♦ Self-care skills
♦ Health care - including medication management
♦ Housekeeping skills
♦ Meeting nutritional needs
♦ Money management skills
♦ Interpersonal skills
♦ Vocational/educational pursuits
♦ Use of leisure time
♦ Time management
♦ Community participation including social networking and utilization of services and resources

PSYCHIATRIC REHABILITATION
Psych-Rehab assists adults 18 years and older who are recovering from a serious mental illness. This program provides several services to assist individuals in choosing, getting and keeping goals of their choice in the educational, vocational, social, living and self-maintenance areas. A large part of this service is the peer support gained – other individuals have walked in your shoes. ‘Empower Self-Support Others’ is a daily goal at this program.
PROGRAM PHILOSOPHY: Psych-Rehab will assist individuals with psychiatric disabilities to decrease impairments for successful community integration. Basing services on strengths and wellness, we are dedicated to providing skill development and support to facilitate the recovery process, while instilling hope, choice, and respect.
